BHNewCo

A designed joint venture became a weekly operating layer — driving patient engagement up 68%.

A behavioral-health network was getting referrals but losing patients. A three-person team turned it into a weekly operating system, and engagement rose 68%.

01

The situation

An established behavioral-health network was receiving 300 to 400 patient referrals a month, but too few became visits. Behavioral care breaks down most often at the handoff: a patient is referred, an appointment may be offered, and the connection is lost. In October 2025, only 22.6% of referred patients actually made it into care. The problem wasn’t getting more referrals. It was converting the ones it already had.

02

The insight

The network didn’t need more referrals, a new directory, or a bigger team. It needed management. The same team that had spent six months designing the joint venture, breaking a 30-year behavioral-health stalemate, already held every decision, data point, and relationship in a single structured context file. So instead of handing off a plan and walking away, that team ran it: decisions Monday, status report Friday, every week.

03

What we built

Design phase: 3 fractional experts instead of a 12-person traditional team. That meant 840 hours instead of 4,240, 14 weeks instead of 20, and 77% lower cost. Operating phase: a six-part operating layer. Coaching the providers who drive referrals. Managing appointment capacity like inventory. Running the funnel from referral to first visit as one job instead of five handoffs. Platform administration. Governance reporting. And a monthly review that separates real movement from noise.

04

What compounded

Four providers drove 73% of referrals, so attention went there first. Dedicated appointment slots grew more than sixfold, from 391 to 2,489 a month. Patients booked into those slots engaged at 45%, versus 26% for everyone else. By February, engagement crossed 36% and held for seven straight months, reaching 37.9% in August. Referrals actually fell 25%, and patients in care rose 25%. Follow-up within 30 days of discharge now runs 36 points above the national average.

What the engine did, and what people did.

The engine

Scored providers every quarter in 75–80% less time, and replaced slide decks with a live dashboard refreshed in minutes.

The people

Built the relationships, coached the providers, and made the calls.

05

What BHNewCo owns

The dashboard, the scoring engine, the monthly review, and a 74-week written record that now serves as institutional memory. Not a folder of decks. A working operating system, built to carry the network the rest of the way to its operational goals.

This operating model doesn’t depreciate. It compounds.

AI-native companies are already operating at a different level – perceiving opportunities others miss, moving faster than the market expects, compounding advantage every week. The ones winning aren’t using better tools. They’re working with better context.

We build your AI-native engine in sprints – strategy, brand, GTM, or operating model – and leave you owning it. Not plans. A running system. If you’re ready:
